Description:
Under the direction of the Clinical Team Manager, you will provide skilled care and case management to patients and families.
This role operates within established organizational and departmental policies and procedures.
You will adhere to the State Nurse Practice Act and accepted professional standards of practice.
The RN case manager will utilize nursing theory and processes in their practice.
You will communicate and document observations and assessments effectively.
Furthermore, you will promote and maintain the health of individuals through direct care, teaching, counseling, and appropriate rehabilitative measures.
