Key Responsibilities of the RN - Case Management - Travel- Perform comprehensive assessment of high-risk members.
- Collaborate with primary care providers to implement individual care plans.
- Address and reassess cost-effective, appropriate levels of care.
- Coordinate non-medical support to ensure treatment compliance.
- Coordinate care transitions with healthcare team.
- Engage specialty resources for optimal patient outcomes.
- Maintain detailed records of clinical and functional outcomes.
- Identify opportunities for health promotion and illness prevention.
- Act as patient advocate protecting privacy and confidentiality.
Requirements of the RN - Case Management - Travel- Current, unrestricted GA License or compact licenses.
- Associates or Bachelor's Degree in Nursing or related field.
- Case Management certification.
- Experience with HCHB, OASIS, and home health.
- Ability to analyze complex medical information and make rational decisions.
