Position Overview:
We are seeking an experienced and compassionate Registered Nurse (RN) Case Manager to coordinate and manage patient care across the healthcare continuum. The RN Case Manager will work closely with physicians, healthcare providers, patients, and families to develop individualized care plans, ensure quality outcomes, and promote efficient utilization of healthcare resources.
Key Responsibilities:
Assess patients' medical conditions, care needs, and treatment plans.
Develop, implement, and evaluate individualized patient care plans.
Coordinate patient care with physicians, specialists, healthcare teams, and other providers.
Monitor patient progress and ensure appropriate delivery of healthcare services.
Conduct patient assessments and reassessments to determine ongoing care needs.
Facilitate hospital admissions, discharges, and transfers when required.
Coordinate post-acute care services, including home health, rehabilitation, and skilled nursing facilities.
Advocate for patients and families to ensure access to appropriate healthcare resources.
Monitor utilization of healthcare services and ensure cost-effective care delivery.
Maintain accurate and timely clinical documentation in accordance with facility policies.
Ensure compliance with California nursing regulations, CMS guidelines, and applicable healthcare standards.
