Job Title: Registered Nurse Case Manager Profession: Nursing Specialty: Case Management Duration: 13 weeks Shift: Day Hours per Shift: 10 hours Experience: Minimum of 1 year required License: Must have a valid RN license Certifications: BLS required; ACM certification preferred Must-Have: Epic experience, Acute Care experience Description: The Case Manager RN is responsible for managing patient care throughout the hospital stay. Candidates must have an active RN license to be considered for employment. This position requires a commitment to providing high-quality nursing care. Weekend shifts are required every other weekend in a four-week schedule. Floating within the scope of case management services to meet facility needs may be required. Candidates should possess strong skills in acute hospital settings, discharge planning, and care coordination. An understanding of CMS guidelines and HIPAA regulations is important. Experience with Microsoft Office and Epic software is necessary. Attendance and punctuality are essential for maintaining patient care standards.
