Key Responsibilities of the RN - Case Management- Perform comprehensive assessments of high-risk members
- Collaborate with primary care providers to implement individualized care plans
- Address and reassess cost-efficient, appropriate levels of care
- Coordinate non-medical support to ensure compliance with treatment plans
- Coordinate care transitions as needed
- Engage specialty resources to achieve optimal outcomes
- Maintain detailed records of clinical, functional, and fiscal outcomes
- Identify opportunities for health promotion and illness prevention
- Act as a patient advocate by protecting privacy and confidentiality issues
Requirements of the RN - Case Management- Associates or Bachelor's Degree in Nursing or related field
- Current, unrestricted state Registered Nurse license
- Ability to analyze complex medical information and make rational decisions
- Case Management certification
- Virginia License or Compact License
- Hospice experience is a must; HCHB experience is a plus
