Key Responsibilities of the RN New Hanover- Perform comprehensive assessments of high-risk members
- Collaborate with primary care providers to implement individualized care plans
- Address and reassess cost-efficient, appropriate levels of care
- Implement non-medical support for treatment plan compliance
- Coordinate care transitions and engage specialty resources as needed
- Maintain detailed records and identify opportunities for health promotion
- Act as a patient advocate, ensuring privacy and confidentiality
Requirements of the RN New Hanover- Associates or Bachelor's Degree in Nursing or related field
- Current, unrestricted state Registered Nurse license
- Ability to analyze complex medical information and make rational decisions
- Case Management certification
