Key Responsibilities of the RN - Case Management - Travel- Perform comprehensive assessment of high-risk members
- Collaborate with primary care providers for individualized care plans
- Address and reassess cost-efficient, appropriate levels of care
- Implement non-medical support for treatment plan compliance
- Coordinate care transitions with healthcare professionals
- Engage specialty resources for optimal patient resolutions
- Maintain detailed records of outcomes and management processes
- Identify opportunities for health promotion and illness prevention
- Act as a patient advocate, ensuring privacy and confidentiality
Requirements of the RN - Case Management - Travel- Associates or Bachelor's Degree in Nursing or related field
- Current, unrestricted state Registered Nurse license
- Case Management certification
- Prior HH experience within the last two years, minimum six months
- Proficiency in HCHB, OASIS
- Ability to analyze complex medical information and make rational decisions
