Key Responsibilities of the RN - Case Management - Travel- Perform comprehensive assessment of high-risk members
- Collaborate with primary care providers for comprehensive care plan implementation
- Identify and reassess cost-efficient, appropriate levels of care
- Ensure compliance with treatment plans through non-medical support
- Coordinate care transitions as needed
- Coordinate with healthcare professionals during patient care transitions
- Engage specialty resources for optimal resolution
- Maintain accurate records of clinical, functional, and fiscal outcomes
- Identify health promotion and illness prevention opportunities
- Act as a patient advocate protecting privacy and confidentiality
Requirements of the RN - Case Management - Travel- RN licensure (can accept compact license)
- Associates or Bachelor's Degree in Nursing or related field
- Current, unrestricted state Registered Nurse license
- Case Management certification
- Home Health care experience, HCHB, OASIS required
- Med surg/critical care skills preferred
