Key Responsibilities of the RN - Case Management- Perform comprehensive assessment of high-risk members
- Collaborate with primary care providers to implement individualized care plans
- Reassess cost-efficient, appropriate levels of care continuously
- Put non-medical support in place to ensure treatment plan compliance
- Coordinate care transitions as needed
- Coordinate with various professionals to transition patients to appropriate care levels
- Engage specialty resources to achieve optimal outcomes
- Maintain detailed records of clinical and fiscal outcomes
- Identify opportunities for health promotion and illness prevention
- Protect patient privacy and confidentiality as an advocate
Requirements of the RN - Case Management- Associates or Bachelor's Degree in Nursing or related field
- Current, unrestricted state Registered Nurse license (compact license accepted)
- Ability to analyze complex medical information and make rational decisions
- Case Management certification
