Job Title: RN - Case Management - Travel
Location: Macon, GA
Pay: $2,546.40 weekly
Shifts: Monday-Friday, 8 am to 5 pm, with on-call once every three weeks

This travel contract position focuses on providing comprehensive care in a home health environment, requiring experience with HCHB/OASIS. The role demands a proactive approach to patient management and collaboration with healthcare providers.

Key Responsibilities of the RN - Case Management - Travel- Perform comprehensive assessment of high-risk members
- Collaborate with primary care providers for comprehensive care plans
- Address and reassess cost-efficient, appropriate levels of care
- Implement non-medical support for treatment plan compliance
- Coordinate care transitions with healthcare professionals
- Engage specialty resources to achieve optimal resolution
- Maintain detailed records of clinical and fiscal outcomes
- Identify opportunities for health promotion and illness prevention
- Act as patient advocate, ensuring privacy and confidentiality
Requirements of the RN - Case Management - Travel- Current, unrestricted GA RN License (will accept compact license)
- Experience with wound care, IV care, HCHB, and home health
- Associates or Bachelor's Degree in Nursing or related field
- Ability to analyze complex medical information for decision-making
- Case Management certification
