Shifts: M-F 8am-5pm, OnCall Required: Rotating weekends
Seven Healthcare are seeking an experienced RN - Case Management - Travel professional in Jeffersonville, IN. This role involves comprehensive assessments and care coordination for high-risk members across Clark, Floyd, Harrison, Crawford, Washington, and Orange counties. This onsite position offers an excellent opportunity to work within a structured environment, requiring robust home health skills including wound care and IV management.

Key Responsibilities of the RN - Case Management - Travel- Perform comprehensive assessment of high-risk members.
- Collaborate with primary care providers to implement individualized, comprehensive care plans.
- Address and reassess cost-efficient, appropriate levels of care.
- Coordinate non-medical support to ensure compliance with treatment plans.
- Facilitate care transitions and engage specialty resources as needed.
- Maintain detailed records of clinical, functional, and fiscal outcomes.
- Identify opportunities for health promotion and illness prevention.
- Act as a patient advocate by protecting privacy and confidentiality issues.
Requirements of the RN - Case Management - Travel- Current, unrestricted RN license in IN or compact license accepted.
- Associates or Bachelor's Degree in Nursing or related field.
- Case Management certification preferred.
- Experience with HCHB and skills in wound care, IV, drain care, and G tubes.
- Ability to analyze complex medical information and make rational decisions.
