Key Responsibilities of the RN - Case Management - Travel- Perform comprehensive assessment of high-risk members
- Collaborate with primary care providers to ensure individualized care plans
- Address and reassess cost-efficient, appropriate levels of care
- Coordinate non-medical support for treatment plan compliance
- Manage care transitions and coordinate with healthcare professionals
- Engage specialty resources for optimal resolution
- Maintain detailed records of clinical, functional, and fiscal outcomes
- Identify opportunities for health promotion and illness prevention
- Act as a patient advocate, maintaining privacy and confidentiality
Requirements of the RN - Case Management - Travel- Associates or Bachelor's Degree in Nursing or related field
- Current, unrestricted state Registered Nurse license (NC or Compact)
- Experience in Home Health (HH), HCHB, and Oasis
- Ability to analyze complex medical information and make rational decisions
- Case Management certification
