Key Responsibilities of the RN - Case Management - Travel- Perform comprehensive assessment of high risk members
- Collaborate with primary care providers for care plan implementation
- Address and reassess cost-efficient, appropriate levels of care
- Ensure non-medical support for treatment compliance
- Coordinate care transitions as needed
- Engage specialty resources for optimal resolution
- Maintain detailed records of outcomes during management
- Identify health promotion and illness prevention opportunities
- Act as patient advocate, protecting privacy and confidentiality
Requirements of the RN - Case Management - Travel- Associates or Bachelor's Degree in Nursing or related field
- Current, unrestricted CO Registered Nurse license
- Ability to analyze complex medical information and make rational decisions
- Case Management certification
