Key Responsibilities of the RN - Case Management - Travel- Perform comprehensive assessments of high-risk members.
- Collaborate with primary care providers to implement individualized care plans.
- Continuously reassess cost-efficient, appropriate levels of care.
- Coordinate care transitions and engage specialty resources as needed.
- Maintain detailed records of clinical, functional, and fiscal outcomes.
- Act as a patient advocate, protecting privacy and confidentiality.
Requirements of the RN - Case Management - Travel- RN License for WI
- Certified CPR/BLS
- Wound Care Experience
- Valid Driver's License and Reliable Vehicle
- Valid Car Insurance
- Case Management certification
- Experience in the use of Electronic Medical Records such as HCHB or Matrix Care
