Key Responsibilities of the RN - Case Management - Travel- Perform comprehensive assessments of high-risk members
- Collaborate with primary care providers to implement individualized care plans
- Address and reassess cost-efficient care levels
- Coordinate non-medical support for treatment compliance
- Manage care transitions with healthcare teams
- Engage specialty resources for optimal patient outcomes
- Maintain thorough records of patient outcomes

- Act as a patient advocate to protect privacy and confidentiality
Requirements of the RN - Case Management - Travel- Associate's or Bachelor's Degree in Nursing or related field
- Current, unrestricted state Registered Nurse license
- Case Management certification
- Ability to analyze complex medical information and make rational decisions
- Experience with HCHB, Home Health, OASIS, and IV
