Travel Nurse RN - Case Manager - $2,531 per week in The Bronx, NY

Manage discharge planning for an expected caseload of 17-25 patients on Medical-Surgical or Telemetry floors, collaborating with an interdisciplinary team including physicians and social workers. Facilitate discharges to home, community, or skilled nursing facilities, and coordinate referrals including housing, shelter packets, hemodialysis, and wound VAC services.

Key Responsibilities

• Manage discharge planning for an expected caseload of 17-25 patients on Medical-Surgical or Telemetry floors, collaborating with an interdisciplinary team including physicians and social workers
• Facilitate discharges to home, community, or skilled nursing facilities, and coordinate referrals including housing, shelter packets, hemodialysis, and wound VAC services
• Round on the unit with physicians and the care team to support timely and appropriate discharge planning
• Work independently and collaboratively with patients, families, and the interdisciplinary team to develop and execute discharge plans
• Utilize the Allscripts EMR system to document case management activities
• Maintain active BLS, PRI, and SCREEN certifications throughout the assignment

Required Qualifications

• Active RN license in New York State
• BLS certification required
• PRI and SCREEN certifications required (available online; candidates without current certification should obtain prior to or at the start of assignment)
• Minimum 1 year of inpatient case management experience
• Demonstrated ability to manage a caseload of at least 20 patients independently
• Strong critical thinking, time management, and communication skills

Preferred Experience / Skills

• Prior critical care nursing experience
• Familiarity with CarePort, MCG, and/or InterQual criteria
• Some Utilization Review experience
• Experience with Allscripts or similar hospital EMR systems
