Description:
This position is a full-time Case Manager Nurse role requiring expertise in managing patient care.
The Case Manager will coordinate care processes for patients to ensure effective discharge planning.
The role may require coverage on weekends and holidays, with expectations to be available for every other weekend.
The nurse will work with interdisciplinary teams, including physical therapy, social services, and rapid response teams.
Strong skills in utilizing assessment tools and guidelines for determining medical necessity are essential.
Experience with diverse patient populations, including behavioral health, is beneficial.
