**Job Description:**
- **Position:** Backfill RN - Case Management
- **Schedule:** 8:00 AM - 4:30 PM with Rotating Weekends

**Job Responsibilities:**
- Collaborate with interdisciplinary teams to provide **comprehensive case management**.
- Identify and address the needs of patients and their families.
- Design and implement individualized care plans to achieve optimal patient outcomes.
- Ensure effective communication with healthcare providers and patients throughout care processes.
**Qualifications:**
- **Years of Experience:** 2 years in nursing, preferably in case management.
- Active RN license.
