**Exciting Opportunity for RN Care Manager!**
**Job Description:**
- Join a compassionate team dedicated to making a positive difference in the lives of patients and the community!
- We are seeking two experienced RN Care Managers for a dynamic 13-week assignment.
- Work full time at our main campus in York, Maine, with flexible shift options:
- Variable shifts from 8:30 AM to 4:30 PM
- Four 10-hour shifts from 7:30 AM to 5:00 PM
- No call required, but weekend and holiday rotation is part of the role.
**Key Responsibilities:**
- Conduct thorough assessments of patient needs to identify resources for a smooth transition to the next care setting.
- Lead and collaborate with an interdisciplinary team of healthcare professionals during daily care rounds and planning meetings.
- Utilize excellent verbal, written, and electronic communication skills, including familiarity with hospital software applications.
- Partner with community organizations to arrange durable medical equipment, health insurance authorizations, and transportation for patients.
- Engage in monitoring and managing the length of stay for inpatient populations.
- Apply knowledge of hospital reimbursement methods and quality measures relevant to care management.
**Qualifications:**
- Hold an Associate or Bachelor’s degree in Nursing.
- Bring 3-5 years of clinical experience in medical/surgical nursing, home care, hospital case management, or utilization review.
- Experience in utilization review and discharge planning is essential.
- Preferred certifications from a nationally recognized case management or utilization review organization.
- Must possess active BLS certification, with CCM or ACM certification being a plus.
**Shift Information:**
- Enjoy variable shift options: 8 AM to 4:30 PM or four 10-hour shifts from 7:30 AM to 5:00 PM.
- Weekend and holiday rotation required.
