RN – Case Management | Travel Contract

Location: 1980 Crompond Rd, Cortlandt Manor, NY 10567, United States
Department: Care Coordination
Contract: 12 Weeks, with strong possibility of extension
Start Date: Oct 10, 2026
Openings: 2
Schedule: 4 × 9.5-hour shifts
Hours: Approximately 38 hours/week
Shift: Day Shift
On-Call: No

Position Overview

We are seeking an experienced Registered Nurse (RN) Case Manager to join the Care Coordination team at NYP Hudson Valley Hospital in Cortlandt Manor, NY.

The ideal candidate will have strong acute care hospital experience and recent experience with DCP (Discharge Care Planning/Discharge Planning).

Schedule

The facility ideally prefers:

  • 4 × 10-hour shifts
  • 7:00 AM – 5:00 PM or 8:00 AM – 6:00 PM
  • Every other Sunday
  • Approximately 2 weekends per month

Some schedule flexibility may be available. An alternative schedule may include:

  • 5 × 8-hour shifts
  • 8:00 AM – 4:00 PM or 9:00 AM – 5:00 PM
  • 2 weekends per month
Responsibilities
  • Coordinate patient care and discharge planning.
  • Assess patients' post-acute care needs.
  • Develop and coordinate appropriate discharge plans.
  • Collaborate with physicians, nurses, social workers, patients, families, and other healthcare professionals.
  • Coordinate transitions of care and appropriate post-discharge services.
  • Assist with referrals and placement when needed.
  • Communicate effectively with multidisciplinary teams.
  • Support safe and timely patient discharge.
  • Ensure appropriate documentation and follow-up related to care coordination.
