Job Title: Travel RN Case Manager - Home Health
Profession: Registered Nurse

Description:
Travel RN Case Manager role in Home Health.
Responsible for start-of-care visits, recertifications, discharges, and staff supervision/coordination.
Approximately 2 to 7 patient visits per day, depending on visit type and drive time.
Primarily serving one area, with coverage into surrounding regions.
Average travel distance is approximately 60 miles one way.
Documentation time is included in the workday.
Home Health only, with no Hospice coverage.
Manager is interviewing and making decisions quickly.
Experienced Home Health RNs with OASIS experience and an active Oregon license will be prioritized.
