Description:
The RN Care Manager will oversee a daily caseload of patients in inpatient units with a variety of care needs.
The role involves attending interdisciplinary rounds.
The RN will complete initial assessments and facilitate the development of a plan of care in collaboration with the patient, family, caregiver, and health care team.
There will be consistent communication with medical teams and patients regarding progress of care needs related to post-discharge care, such as home health and inpatient rehab.
The RN will interact with post-acute care referral sources to establish a plan.
Duties include documenting and placing orders for home health, durable medical equipment, and nutritional needs within the electronic medical record system.
