Experience: Minimum of 2 years recent RN Case Management experience, with experience in a long-term acute care or short-term acute care hospital setting.
License: Must have a valid RN license
Certifications: Must have BLS certification through the American Heart Association
Must-Have:
- Minimum of 2 years recent RN Case Management experience
- Experience in a long-term acute care or short-term acute care setting
- Experience with care transitions, discharge planning, and strong interdisciplinary team collaboration
- BLS certification must be attached
Description:
Coordinates and facilitates the care of the patient population through effective collaboration and communication with the interdisciplinary care team members.
Follows patients throughout the continuum of care and ensures optimum utilization of resources and service delivery.
Ensures compliance with external review agencies.
Provides ongoing support and expertise through comprehensive assessment, care planning, plan implementation, and overall evaluation of individual patient needs.
Enhances the quality of patient management and satisfaction.
Promotes continuity of care and cost-effectiveness through the integration of case management functions, utilization review, and discharge planning.
