Key Responsibilities

• Perform case management and discharge planning functions within an acute care hospital setting
• Coordinate patient transitions and facilitate safe, timely discharges
• Collaborate with interdisciplinary teams to assess patient needs and develop discharge plans
• Communicate with patients, families, and care teams to ensure continuity of care post-discharge

Required Qualifications

• Active Massachusetts RN license required; candidates actively in the process of obtaining a permanent MA license (conditional license) will be considered
• Minimum of 2 years of case management experience
• At least 6 months of discharge planning experience in an acute care hospital setting

Preferred Experience / Skills

• Experience in a large organization or teaching hospital with 500 or more beds
• Strong knowledge of discharge planning processes and post-acute care resources
• Ability to adapt to varying schedule structures based on operational needs
