- Ability to multitask and prioritize in a fast-paced environment
- Basic understanding of acute and post-acute levels of care and resources
Description:
The role involves managing a varied caseload of patients, ensuring effective and efficient care coordination throughout the continuum of care.
The Case Manager will collaborate with patients, families, and a multidisciplinary team to develop individualized care plans from admission to discharge.
Daily responsibilities include interdisciplinary rounding and facilitating team conferences.
The position requires participation in utilization reviews, insurance authorizations, and concurrent reviews.
Managing resources and ensuring cost-effective patient care are key components of this role.
Familiarity with complex patient cases, including those requiring respiratory support and intravenous therapies, is preferred.
Candidates will support post-discharge referrals and care resource alignment, including durable medical equipment and rehabilitation service setups.
Strong communication skills and the ability to interact with diverse populations are critical for success in this position.
Flexibility for occasional weekend inquiries may be required.
Business casual or scrubs are acceptable attire for this position.
