Candidates will support post-discharge referrals and care resource alignment, including durable medical equipment and rehabilitation service setups. Strong communication skills and the ability to interact with diverse populations are critical for success in this position. Flexibility for occasional weekend inquiries may be required. Business casual or scrubs are acceptable attire for this position.
