Description

Key Responsibilities
  • Perform care coordination and discharge planning within an acute care hospital setting
  • Manage a caseload with a 1:20 nurse-to-patient ratio
  • Participate in weekend rotation and meet departmental scheduling needs
  • Collaborate with interdisciplinary teams to facilitate appropriate and timely patient transitions
  • Candidates may be expected to float between nearby sister facilities as needed
Required Qualifications
  • Active California RN license required upon start date (pending license accepted at time of submission)
  • Minimum 2 years of acute care hospital Case Management experience, including care coordination and discharge planning - hospital experience is required
  • Compact license must be in hand at time of submission if applicable
  • Must commit to working 3 of the 5 designated holidays: Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, and New Year's Day
  • All RTO and specific scheduling requests must be submitted upfront; post-submission RTO requests may not be considered
  • Must live more than 50 miles from the facility to be eligible as a traveler (locals considered under separate terms)
  • 2 professional references covering at least 1 year within the last 3 years required prior to start
Preferred Experience / Skills
  • Prior travel nursing experience (first-time travelers accepted and will be screened)
  • Familiarity with utilization review and payer requirements in an acute care environment
  • Experience working within large health system environments
