Shift: Day Shift, 8:00 AM – 4:30 PM
Guaranteed Hours: 40 Hours per Week
Experience Required: Minimum 3 Years

Key Responsibilities

• Assess patients' clinical needs and develop individualized care management plans.

• Coordinate patient care with physicians, nurses, social workers, and other healthcare professionals.

• Monitor patient progress and identify barriers to treatment or discharge.

• Facilitate discharge planning and coordinate appropriate post-acute care services.

• Educate patients and families about treatment plans, available resources, and follow-up care.

• Advocate for patients to support quality outcomes and continuity of care.

• Maintain accurate patient records and care management documentation.

• Help coordinate healthcare resources while following established policies and clinical standards.
