TravelNurseSource is working with Malone Healthcare to find a qualified Case Manager RN in Burbank, California, 91501!
Key Responsibilities
Perform care coordination and discharge planning for an acute care patient population
Manage a caseload with a 1:25 ratio on day shift
Collaborate with interdisciplinary teams to facilitate appropriate and timely patient transitions
Participate in weekend coverage, typically one full weekend per four-week period, subject to departmental need
Commit to working three of five designated holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
Required Qualifications
Active California RN license required by start date (pending license accepted at time of submission)
Compact license must be in hand at time of submission, if applicable
Minimum of 2 years of acute medical Care Manager experience in care coordination and/or discharge planning
Minimum of 1 year of relevant experience required per facility compliance standards
Two professional references covering at least 1 year of experience within the last 3 years required prior to start
Must live at least 50 miles from the facility to be eligible as a standard traveler (locals accepted under a separate rate structure)
Preferred Experience / Skills
Prior travel nursing experience (first-time travelers considered with screening)
Familiarity with acute care hospital systems and interdisciplinary care team workflows
Experience with utilization review or managed care in a hospital setting
