Job Title: RN Case Manager Profession: Registered Nurse Specialty: Case Management Duration: 13 weeks Shift: Day Hours per Shift: 10 hours Experience: Minimum of 2 years as an RN and 1 year as a Case Manager License: Current and valid driver's license or state-issued ID required Certifications: BLS required; CCM or ACM preferred Must-Have: - Recent case/resource management experience Description: RN Case Managers support inpatient discharge planning. They assist with care coordination and care progression. They facilitate post-acute transitions and ensure patient choice compliance. They meet regulatory requirements as needed. Candidates must be willing to float to other areas if required. No more than 5 days of requested time off will be accepted for every 13-week assignment. Candidates must submit a utility bill in their name with a matching address that is greater than 75 miles from the contracted facility. All candidates must include two supervisory references with their application. Modules may be required and will be recorded during the orientation process. Candidates must be aware and willing to float to different areas as necessary. Submissions with gaps in the profile or with any license infractions will not be considered.
