This role focuses on working in the Emergency Department for a readmission initiative. Key Responsibilities of the RN - Case Management - Travel- Collaborate with the healthcare team to manage patient readmission initiatives in the ED
- Assess, plan, and coordinate patient care to ensure seamless transitions
- Engage in floating duties, potentially covering other campuses as required
- Function effectively within a multidisciplinary team to deliver high-quality patient care
Requirements of the RN - Case Management - Travel- FL/Compact nursing license (recent Nursys)
- 2 years of Case Manager experience, acute care experience required
- BLS certification
- Prior travel experience required
- Degree accreditation on profile or 5 years of experience
