This role focuses on working within the Emergency Department as part of the readmission initiative. Key Responsibilities of the RN - Case Management- Participate in the readmission initiative in the Emergency Department
- Function effectively as a core staff within units
- Coordinate care and ensure smooth transitions for patients
- Float to other sister campuses as required
Requirements of the RN - Case Management- 2 years of Case Management experience required
- Acute care experience required
- Basic Life Support (BLS) certification required
- Prior travel experience required
- FL/Compact nursing license required (recent Nursys must be on profile)
- Degree accreditation must be on profile or 5 years of experience
