Minimum 2 years of acute inpatient case management experience required.

Traveler experience of 2 years required.

Current RN licensure permitting work in the state and Basic Life Support (BLS) certification required.
Certifications:
Advanced life support certification (ACLS and PALS) may be required in specific departments.
Must-Have:
EPIC electronic medical record experience required.
No large gaps in work history during the assignment period.
Ability to cover at least 3 weekend days during the contract period.
Description:
The candidate will be responsible for all aspects of the transitional plan of care from admission to discharge.
They should be comfortable conducting initial case management assessments and providing transitional care follow-up.
Ensuring safe discharge to the patient's final destination, whether that be home, self-care, home health, skilled nursing facility, or inpatient rehabilitation is essential.
Candidates must be highly skilled, able to multitask, prioritize, document accurately, and collaborate effectively with community partners.
Knowledge of Medicare, Medicaid, and insurance coverage is beneficial but not required.
The position does not include providing mental health therapy; candidates will assess cases and refer to specialized teams as needed.
The daily patient population includes adults and geriatrics, with an average census of 525.
Interdisciplinary support includes physical therapy, respiratory services, interpretation services, radiology, social services, pharmacy, and transportation.
The unit provides care for cardiac, surgical, medical/surgical, and neurological diagnoses among others.
Candidates must be prepared to work within a dynamic and diverse patient care environment.
