Description:
The position is for a Case Manager RN.
This role requires managing all medical and surgical patients.
Work is scheduled from Monday to Friday.
No weekends or holidays are required.
A daily census of approximately 25 patients will be managed.
Candidates must demonstrate skills in care coordination and case management.
Experience in admission criteria, appeals and denials, and discharge planning is essential.
Knowledge of CMS guidelines and medical necessity determination is necessary.
Candidates should be comfortable with various patient age groups, including adults and geriatrics.
The role mandates adherence to HIPAA guidelines and knowledge of documentation requirements.
Candidates must be comfortable floating within the scope of practice.
The position involves close collaboration with interdisciplinary support teams.
