Job Title: Case Manager Profession: Registered Nurse Specialty: Case Manager Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 16:30) Experience: Minimum of 2 years of recent experience required within the last 7 years License: Active RN license required Certifications: BLS required Must-Have: Acute hospital experience required Description: The Case Manager will accept either Registered Nurses or Social Workers. An ASAP start is required. Local candidates within a 60-mile radius will be considered. The position requires experience in acute rehabilitation, discharge planning, interdisciplinary collaboration, and care coordination. Experience in acute hospital settings is a must. Strong skills in medical necessity and discharge planning are essential. Candidates must be knowledgeable about CMS, HIPAA guidelines, and clinical documentation. Candidates should be experienced with care coordination and utilization management processes. Two supervisory references are required that support at least 2 years of experience in the previous 3 years. No requested time off is permitted during the first two weeks of the assignment. RTO must be included at the time of submission and cannot be changed after submission. Candidates must plan ahead for any compliance requirements and orientation modules.
