- Ability to assess, evaluate, and drive discharge planning
- Experience in care coordination
- Knowledge of Medicare regulatory guidelines
Description:
Responsible for assessing, evaluating, and driving the discharge plan.
Coordinates services to the patient's appropriate level of care.
Ensures that treatment plans are initiated, evaluated, and re-evaluated daily.
Follows the progress and lack thereof on a daily basis.
Monitors length of stay and identifies barriers to discharge.
Follows regulatory guidelines regarding patient choice and notices.
Scheduled to work Monday through Friday with no weekend coverage required.
Floating within facility and scope of practice as needed.
Compliance with all required onboarding documentation is mandatory prior to starting.
No request for time off during orientation week.
If one major holiday falls during the assignment period, the traveler is expected to work it.
If two major holidays fall during the assignment period, the traveler is expected to work both.
If three major holidays fall during the assignment period, the traveler is expected to work two.
All requests for time off must be finalized at the time of submission.
