Description:
Ability to assess, evaluate, and drive discharge plans.
Coordinate services to the patient's appropriate level of care.
Ensure that treatment plans are initiated, evaluated, and re-evaluated daily.
Monitor progress and identify any barriers to discharge.
Follow regulatory guidelines regarding patient choice and notices.
Expected to work during major holidays if assigned.
No requests for time off during orientation week.
All time-off requests must be finalized at the time of submission.
Full compliance required to start on time.
Local candidates accepted at the same rate.
Experience with patient ratios and charting systems preferred.
Experience with acute hospital and long-term care settings desired.
Familiarity with Medicare guidelines and care coordination practices essential.
