Description:
This role involves the assessment, evaluation, and management of patient discharge plans.
The registered nurse will coordinate services to ensure patients receive the appropriate level of care.
Daily evaluation and re-evaluation of treatment plans are essential.
Monitoring patient's progress, length of stay, and identifying barriers to discharge are also key responsibilities.
Familiarity with Medicare guidelines regarding patient choice and notifications is required.
The schedule is from Monday to Friday with no weekend or call responsibilities.
Candidates must have previous travel experience and be familiar with charting systems.
Compliance with all onboarding requirements is mandatory.
