Job Title: Care Manager

Experience:
- Minimum of five (5) years of clinical experience as a Registered Nurse.
- Experience with care coordination, utilization review, and discharge planning.
License:
- Licensed as a Registered Nurse in the State of New York.
- Primary source verification of license and registration from the NYS Department of Education.
Certifications:
- Basic Life Support (BLS) certification.
- NYSDOH H/C PRI training certificate required by submission or obtained post-offer.
- PRI and Screen certification within three months of employment is required.
Must-Have:
- Bachelor's Degree from an accredited nursing program is preferred.
Description:
