Travel Nurse RN - Case Manager - $2,326 per week in Fort Defiance, AZ

Key Responsibilities

• Perform case management and coordinate the discharge planning care process for age-appropriate inpatients with chronic diseases and special health care needs across the Multi-Service Unit, Intensive Care Unit, OB Units, and Adolescent Care Unit
• Maintain a tracking system for referred patients for admission with other healthcare facilities
• Conduct chart reviews for requested services related to return-to-facility or discharge-home planning
• Attend staff meetings as scheduled, including mandatory New Employee Orientation
• Maintain compliance with clearance requirements; travelers must be cleared 7 days prior to their start date

Required Qualifications

• Minimum of two (2) years of experience in discharge planning and Med-Surg
• RN experience in a clinical, medical/surgical, acute, and/or critical care setting required
• Valid and unrestricted Registered Nurse (RN) license in any U.S. State or Territory
• Valid American Heart Association (AHA) Basic Life Support (BLS) certification, must be maintained throughout assignment

Preferred Experience / Skills

• Experience working across multiple inpatient units including ICU, OB, and adolescent care settings
• Familiarity with coordinating care transitions and inter-facility referral processes
