Description:
Ability to assess, evaluate, and drive discharge plans.
Coordinate services to the patient's appropriate level of care.
Ensure that the treatment plan is initiated, evaluated, and re-evaluated daily.
Follow the progress and lack thereof daily.
Monitor length of stay and identify barriers to discharge.
Follow regulatory guidelines regarding patient choice, IMM notices, and MOONS.
SCHEDULE: Monday through Friday.
Weekend: None.
Call: None.
Floating: Within facility/scope of practice as needed.
Clinical screening: Candidates will be reviewed by clinical to determine fit.
If clinical pass and within guidelines, candidates will be sent to the manager for final decision.
Clinical screens will be sent on a case-by-case basis.
RTO: Maximum of 7 days, please discuss requests during the interview.
Holidays: If one major holiday falls within the assignment period, the traveler is expected to work it.
If two major holidays fall within the assignment period, the traveler is expected to work both holidays.
If three major holidays fall within the assignment period, the traveler is expected to work two of them.
Major holidays include: Memorial Day, Independence Day, Labor Day, Thanksgiving/Black Friday, Christmas Eve/Christmas Day, New Year's Eve/New Year's Day.
Compliance: Full compliance is required to start on time; if not compliant in time, the push back is 2 weeks due to the bi-weekly orientation schedule.
Local travelers accepted at the same rate.
All RTO must be finalized at the time of submission.
No RTO can be requested during the orientation week (Monday to Sunday).
Maximum RTO is 7 days per contract.
If one or two holidays fall during the assignment, no holiday RTO can be requested.
If three holidays fall during the assignment, one holiday RTO may be considered.
Onboarding compliance items/labs must be completed within 48 hours of contract signing.
All compliance documents are due by Tuesday prior to the start date.
