Job Title: RN Case Manager Profession: Registered Nurse Specialty: Home Health Duration: 13 weeks Shift: Days Hours per Shift: 32 Experience: Minimum of 2 years in Home Health License: Licensed Registered Nurse Certifications: Graduate of an accredited school of nursing Must-Have: Licensed Registered Nurse in the applicable states. Minimum of 2 years of experience in a Home Health position. Description: This is a Full-Time RN Case Manager position in Home Health. You will develop strong relationships with your patients and be an integral part of their care team. You will have considerable independence while being part of a cohesive patient-care team. The role offers a work/life balance with the necessary flexibility. You will screen, evaluate, re-evaluate, and treat patients while coordinating and communicating treatment activities with interdisciplinary team members and patients. Upon admission, you will develop a care plan in collaboration with the patient's physician, the patient, and their family or primary caregivers. You will periodically review and recertify the patient care plan as appropriate. You will coordinate the patient's care plan through communication and case conferences with the interdisciplinary team. You are responsible for timely advising other disciplines of discharges. You will provide ongoing assessments of the patient's clinical, functional, and emotional status to identify problems or progress towards desired goals. You will communicate significant changes to the patient's physician and colleagues. Documentation of evaluations, treatment goals, and objectives is required. You will teach patients and caregivers self-care techniques as appropriate. You will provide medication, diet, and other instructions as ordered by the physician and utilize opportunities for health counseling with patients and caregivers.
