Key Responsibilities

• Provide case management services within an acute care hospital setting
• Coordinate patient care plans and facilitate transitions of care across the continuum
• Float between like units based on facility need
• Document and chart patient information using Epic EMR
• Work every other weekend as required by the facility
• Commit to working 3 of the 5 designated holidays: Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, and New Year's Day

Required Qualifications

• Active RN license required; compact license must be in hand at time of submission
• Minimum of 2 years of nursing experience required, with acute care experience mandatory
• At least 1 year of relevant experience required per facility compliance standards
• 2 professional references covering at least 1 year of experience within the last 3 years required prior to start
• Candidates must submit all requested time off (RTO) and scheduling requests at the time of initial submittal - post-offer RTO may not be considered or granted
• Local candidates are welcome; candidates residing within 50 miles of the facility are considered local

Preferred Experience / Skills

• Prior experience in hospital-based case management
• Familiarity with Epic EMR
• Experience floating between units in an acute care environment
