Description:
We are seeking an RN Care Manager to bring comfort, compassion, and quality care to patients and families when they need it most.
The RN Care Manager's ability to manage patient care and provide services requires substantial specialized bedside nursing skills for symptom management, crisis intervention, and family education.
Excellent written and verbal communication, problem-solving, and decision-making abilities are essential.
Candidates should demonstrate experience in developing and maintaining rapport with patients and families and the ability to work well with an interdisciplinary team.
Working conditions primarily involve traveling to patients' residences.
Travel to long-term care facilities, hospitals, and area agencies and businesses will be necessary.
Overnight travel outside of the service area may be necessary for continuing education.
Employees are at direct risk for exposure to infections and blood-borne pathogens.
