Description:
We are seeking an RN Care Manager to bring comfort, compassion, and quality care to patients and families when they need it most.
As an RN Care Manager, your ability to manage patient care and provide services requires substantial, specialized bedside nursing skills for symptom management, crisis intervention, and family education.
In addition to excellent written and verbal communication, problem-solving, and decision-making skills, candidates should demonstrate experience in developing and maintaining rapport with patients and families.
Candidates must also possess the ability to work well with an interdisciplinary team.
The position primarily involves working in patients' residences, with necessary travel to various care facilities and agencies.
Overnight travel outside of the service area may be required for continuing education.
Employees may be at direct risk for exposure to infections and blood-borne pathogens.
