We are seeking an experienced Case Management Registered Nurse (RN) to join a public health / community health (ykhc) in Bethel, AK. In this role, you will coordinate comprehensive patient care plans, facilitate safe and timely discharges, and serve as a key liaison between patients, families, and the multidisciplinary care team. Key Responsibilities
• Assess patients' medical, psychosocial, and functional needs to develop individualized care coordination plans
• Collaborate with physicians, social workers, and interdisciplinary teams to facilitate appropriate level-of-care transitions
• Coordinate timely discharge planning to ensure safe transitions to post-acute or community-based settings
• Identify and address barriers to discharge, including social determinants of health and insurance authorization needs
• Conduct utilization review and monitor patient status to support appropriate admission criteria and length of stay
• Educate patients and families on discharge instructions, community resources, and follow-up care plans
• Document all case management activities accurately in the electronic health record in accordance with regulatory standards
✅ Requirements
• Active AK or Compact RN license
• BLS, ACLS, PALS certifications
• 2+ years of Case Management experience
• Case Management certification (ACM or CCM) preferred
Pay
Pay range: $2,208 to $2,300 per week.
Travel Case Manager RN package: up to $2,300/week (32 hours) = $41/hour taxable + up to $988/week in travel reimbursements if eligible under IRS tax-home and expense rules.
Local package: $69/hour fully taxable ($2,208/week for 32 hours).
Overtime paid in accordance with applicable wage-and-hour laws and the assignment agreement. Actual compensation varies based on hours worked, assignment terms and travel reimbursement eligibility.
