Schedule: Monday through Friday, 8:00 AM – 4:30 PM, with potential weekend coverage as neededHighlights:

• Manageable patient ratio of 1:21 allowing for focused case management and discharge planning
• Utilize Meditech and CarePort documentation systems for comprehensive care coordination
• Conduct daily case management assessments and evaluations across various service lines

Requirements:

• Active Registered Nurse (RN) license in Florida or compact state eligibility
• 1–2 years of nursing experience required
• Previous Case Management experience strongly preferred
• Certified Case Manager (CCM) credential preferred but not required
• Proficiency with computer systems and documentation platforms
• Strong assessment, evaluation, and care coordination skills
