Must-Have:
- Experience in care coordination and discharge planning
- Knowledge of Medicare regulatory guidelines
- Familiarity with clinical screening procedures
- Capability to monitor length of stay and identify discharge barriers
Description:
Ability to assess, evaluate, and drive discharge plans.
Coordinate services to the patient's appropriate level of care.
Ensure that treatment plans are initiated, evaluated, and re-evaluated daily.
Follow the progress and lack thereof daily.
Monitor length of stay and identify barriers to discharge.
Adhere to regulatory guidelines regarding patient choice and notices.
Work Monday through Friday with no weekend or call requirements.
Must work one major holiday if it falls within the assignment period.
If three major holidays occur during the assignment, two must be worked.
Maximum of seven days of request time off allowed, to be discussed during the interview.
Travel experience is required for this position.
Clinical reviews will determine the candidate's clinical fit for the role.
