Exciting Opportunity for RN Case Manager - Travel Position! Are you ready to embark on a rewarding journey as an RN Case Manager? Join a dynamic team that is committed to delivering exceptional patient care while exploring new horizons! Provide comprehensive case management for acute hospital patients. Coordinate and facilitate patient care plans effectively. Utilize Meditech, Midas, and Interqual for streamlined operations. Collaborate with healthcare teams to ensure optimal patient outcomes. Engage with patients and their families to provide education and support. Participate in quality improvement initiatives within the facility. 2 years of acute (hospital) Case Management experience. Knowledgeable in Meditech, Midas, and Interqual. Must be available to work every other weekend. Strong communication and interpersonal skills.
