Job Title: Case Manager Profession: Registered Nurse Specialty: Care Management Duration: 11 weeks Shift: Day Hours per Shift: 8 hours Experience: Minimum 3 years License: GA or Compact license required Certifications: BLS Must-Have: Previous charge experience; travel experience required; clinical fit assessment required. Description: Ability to assess, evaluate, and drive discharge plans. Coordinate services to the patient's appropriate level of care. Ensure that the treatment plan is initiated, evaluated, and re-evaluated daily. Follow the progress and address any lack of progress daily. Monitor length of stay and identify barriers to discharge. Follow regulatory guidelines regarding patient choice and notifications. Work Monday through Friday, with no weekend or call requirements. Floating within facility/scope of practice as needed. The candidate will undergo a clinical review to determine fit. Maximum 7 days of pre-approved time off can be discussed during the interview. Work requirements include potential holiday shifts as per the outlined policy. Full compliance with onboarding requirements is necessary to start on time. Completion of core items and labs is required within specified timelines. All requests for time off must be finalized at the time of submission.
